A health care professional must provide evidence of medical necessity for the cost of mouth wash to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement.

Mouth Guard

A health care professional must prescribe Over-the-Counter (OTC) drugs and medicines for you to be reimbursed for the expense. With a prescription, the cost of an mouth guard is an eligible medical expense.

Menstrual Care Products

Menstrual Care Products, such as tampons, pads, liners, cups, sponges, or similar products for menstruation are eligible expeneses for HSA and FSA.

Costs associated with medical services are an eligibile expense. This can include fees paid to a licensed physician for treatment of a medical condition, phone consultations such as TeleDoc, and virtual office visits with a licensed physician.
