Costs associated with medical care received outside of the United States are eligible expenses.
Maternity Expenses
Maternity costs associated with the services of a physician or licensed midwife for the delivery of your child are eligible expenses.
Mastectomy Related Expenses
The following expenses associated with a mastectomy are eligible medical expenses:
Breast prosthesis
Bras or other clothing specifically designed to hold the breast prosthesis (eligible amount must be above cost of the product in its standard form)
Specials bras or other clothing with a built-in prosthesis
Long Term Care Services
Costs associated with long term care services that are limited to the medical portion of care are considered eligible expenses.
Lice Removal
Any fees paid to a lice treatment specialist for lice removal are an eligible expense.
