Hair Removal / Transplant
A health care professional must provide evidence of medical necessity for the cost of hair removal or transplant to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement.
Gynecologist
The cost of gynecological care is an eligible medical expense.
Gym Membership Fees
Gym or health club membership dues paid for general health and not related to a medical condition are not eligible medical expenses.
Guide Dog
The purchase cost along with any amounts paid for the care of a guide dog or other animal used by a visually or hearing impaired person, or to assist a person with another disability, is an eligible medical expense.
