Maternity clothes are not considered an eligible expense.
The following expenses associated with a mastectomy are eligible medical expenses:
Breast prosthesis
Bras or other clothing specifically designed to hold the breast prosthesis (eligible amount must be above cost of the product in its standard form)
Specials bras or other clothing with a built-in prosthesis
A health care professional must provide evidence of medical necessity for the cost message therapy to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement.
Costs associated with long term care services that are limited to the medical portion of care are considered eligible expenses.
