Note: Other expenses associated with an active attempt to conceive (i.e. amounts paid for egg and sperm donation, treatment of infertility, and semen and embryo storage (including shots, in-vitro fertilization, and artificial insemination) must be recommended by a healthcare professional and will require a Letter of Medical necessity to be eligible.
The cost of eye exams, eye patches, reading glasses including OTC reading glasses, prescription eyeglasses (including prescription sunglasses), tinting of prescription eyewear, prescription goggles and prescription safety glasses are eligible medical expenses. In addition, repairs to Eyeglasses, Prescription Sunglasses, Reading Glasses and Prescription Goggles/Safety Glasses are all eligible medical expenses.
Note: Non-prescription sunglasses, eyeglasses needed for vision correction or reading as well as warranties for repairs and replacements are not eligible medical expenses.
