Cosmetic Treatment

A health care professional must provide evidence of medical necessity for the cost cosmetic surgery for improving a deformity arising from, or directly related to, a congenital abnormality, personal injury or disfiguring disease to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement.

Contact Lenses and Supplies

Contact lenses needed to correct vision, amounts paid for the purchase of contact lenses (including shipping and handling fees) are eligible medical expenses.In addition, amounts paid for contact lens solution, supplies and exams (including fittings) are also eligible expenses.
Note: Amounts paid for cosmetic contact lenses (such as to change eye color without vision correction) are not eligible medical expenses.
