Note: The cost of an abdominoplasty such as a Tummy Tuck is not eligible if the sole purpose is to enhance appearance.

Eligible w/LMN Air Conditioners/Air Purifiers

A health care professional must provide evidence of medical necessity for the cost of a portable or permanent unit to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Allergy Treatment Products

A health care professional must provide evidence of medical necessity for the cost of a alergy treatment products to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

If the product could be owned even without allergies (such as a pillow or vacuum cleaner), the expense is generally not an eligible expense.

Eligible w/LMN Alopecia Treatment (Baldness)

Alopecia is a Medical Condition resulting in the loss of hair from the body or head that can lead to baldness. A health care professional must provide evidence of medical necessity for the cost of a Alopecia treatment to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

This condition can also be attributable to normal male pattern baldness. Treatment for this is not eligible if the sole purpose is to enhance appearance. 

Eligible w/LMN Alternative Healers or Treatments

Non-traditional healing treatments provided by professionals may be eligible if provided to treat a specific medical condition. The treatments must be legal and the expenses do not qualify if the remedy is a food or substitute for food that the person would normally consume in order to meet nutritional requirements. Drugs and medicines recommended by alternative healers to treat a specific medical condition also can qualify as medical care, however as of December 31, 2010 a health care professional must provide evidence of medical necessity for these treatments to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Artificial Reproductive Technologies

A health care professional must provide evidence of medical necessity for the cost of Artificial Reproductive procedures to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

To qualify as potentially eligible, procedures must be intended to overcome an inability to have children due to medical reasons and are performed on you, your spouse or your dependent. Eligible medical expenses include (but are not limited to):

  • Fertility exams
  • Artificial insemination (intracervical, intrauterine, intravaginal)
  • In-vitro/In-vivo fertilization
  • Gamete Intrafallopian Transfer (GIFT)
  • Sperm bank storage/fees for artificial insemination. *Note: Storage fees can only be submitted for planned fertilizations during the current plan year.
  • Sperm implants
  • Sperm washing
  • Reverse vasectomy
    • Embryo replacement and storage.

  • Egg donor charges for recipient
  • Embryo transfer
Eligible w/LMN Aspirin

A health care professional must prescribe Over-the-Counter (OTC) drugs and medicines for you to be reimbursed for the expense. With a prescription, the cost of OTC Aspirin medicine is an eligible medical expense. 

Eligible w/LMN Baby Formula

A health care professional must provide evidence of medical necessity showing the infant requires special dietary requirements, and a specific baby formula is recommended to treat an illness or disorder to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Note: If your baby requires a special formula to treat an illness or a disorder, the difference in cost between the special formula and routine baby formula can be reimbursed.

Eligible w/LMN Baldness

A health care professional must provide evidence of medical necessity for the cost of the medical treatment to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Note: Treatment for normal male pattern baldneess is not eligible if the sole purpose is to enhance appearance. 

Eligible w/LMN Birthing Coach

A health care professional must provide evidence of medical necessity for the cost of a professional birthing coach who provides medical care for the mother or child to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

 

Eligible w/LMN Blood Donor Fee

A health care professional must provide evidence of medical necessity for the cost of blood donation to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Note: A fee associated with blood donation, when the blood is for the subsequent use by the participant, spouse or eligible dependent, is an eligible medical expense. 

Eligible w/LMN Blood Storage

A health care professional must provide evidence of medical necessity for the cost of blood storage to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. Storage fees should not exceed six months.

Note: As long as a medical condition is present, the cost to store blood would be an eligible medical expense. This included the cost of collecting, freezing and storing umbilical cord blood.

Eligible w/LMN Breast Implants or Removal

A health care professional must provide evidence of medical necessity for the cost of breast implants or removal to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Breast Reduction

A health care professional must provide evidence of medical necessity for the cost of a breast reduction to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Note: A breast reduction for the sole purpose of improving appearance is not an eligible expense.

Eligible w/LMN Capital Expense

A health care professional must provide evidence of medical necessity for the cost of installing equipment in the home to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

A capital expense (permanent or portable) can be reimbursed if its purpose is to provide medical care for you, your spouse or dependent. Expenses for improvements or special equipment added to your home can be reimbursed if the main purpose of the item is medical care. How much is reimbursed depends on the extent to which the expense permanently improves the property and whether others benefit.

Note: The amount paid for the improvement is reduced by the increase in the value of your home or property. The difference between the cost of the improvement minus the increased value equals the eligible expense. Also see Publication 502 at www.irs.gov for more information including a worksheet.

Eligible w/LMN Car Modification

A health care professional must provide evidence of medical necessity for the cost of car modifications such as hand controls and other special equipment installed in a car including installation costs to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Note: The cost of operating a specially equipped car is not an eligible medical expense.

Eligible w/LMN Childbirth

Amounts paid to a physician or licensed midwife for delivery charges are eligible medical expenses. Additonal costs related to certain maternity items such as products that reduce discomfort associated with pregnancy (i.e. maternity girdle and maternity support belts) are also eligbile expenses.

Eligible w/LMN Colon Therapy, Colon Hydrotherapy, Colonics

A health care professional must provide evidence of medical necessity for the cost of colon therapy to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

 

Eligible w/LMN Cosmetic Treatment

A health care professional must provide evidence of medical necessity for the cost cosmetic surgery for improving a deformity arising from, or directly related to, a congenital abnormality, personal injury or disfiguring disease to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

 

Eligible w/LMN Diabetic Shoes

If used to treat or alleviate a specific medical condition, only the excess cost of the specialized shoes over the cost of regular shoes will qualify. However, a health care professional must provide evidence of medical necessity for the cost of diabetic shoes to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

 

 

Eligible w/LMN Diapers, Diaper Service

A health care professional must provide evidence of medical necessity for the cost of diapers and diaper service to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

 

Eligible w/LMN Doulas

If the doula who provides medical care for the mother or child is a licensed health care professional who renders medical care, then their fees can be reimbursed. However, a health care professional must provide evidence of medical necessity for the cost of a Doula to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Educational Classes

A health care professional must provide evidence of medical necessity for the cost of an educational class specific to a medical condition such as learning to manage diabetes or learning Braille to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Egg and Embryo Storage Fees

A health care professional must provide evidence of medical necessity for the cost of eggs and embryo storage to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Egg and Sperm Donor Fees

A health care professional must provide evidence of medical necessity for the cost of egg and sperm donor fees to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Excess Skin Removal

A health care professional must provide evidence of medical necessity for the cost of excess skin removal to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Note: Removal of excess skin soley for the enhancement of appearance is not eligible.

Eligible w/LMN Exercise Equipment, Exercise Program

A health care professional must provide evidence of medical necessity for the cost of exercise equipment or program to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and expense is for treatment of a medical condition with the request for reimbursement. 

Note:The cost of exercise equipment or an exercise program for improvement of general health is not an eligible medical expense.

Eligible w/LMN Hair Removal / Transplant

A health care professional must provide evidence of medical necessity for the cost of hair removal or transplant to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Homeopathic Care

A health care professional must provide evidence of medical necessity for the cost of homepathic care to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement. Treatment must also be perfromed by a licensed healthcare professional.

Eligible w/LMN Humidifier

A health care professional must provide evidence of medical necessity for the cost of a humidifier to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Hydrotherapy

A health care professional must provide evidence of medical necessity for the cost of hydrotherapy to be an eligible medical expense. Eligible w/LMN Hypnosis

A health care professional must provide evidence of medical necessity for the cost of hypnosis to be an eligible medical expense. Eligible w/LMN Learning Disabilities

A health care professional must provide evidence of medical necessity for the cost of tuition/tutoring fees covering services rendered specifically for your child's severe learning disabilities caused by mental or physical impairments to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Massage Therapy

A health care professional must provide evidence of medical necessity for the cost message therapy to be an eligible medical expense. Include evidence of medical necessity by providing a Letter of Medical Necessity (LMN) that specifically identifies the recommendation and that the expense is for treatment of a medical condition with the request for reimbursement. 

Eligible w/LMN Mouth Wash

A health care professional must provide evidence of medical necessity for the cost of mouth wash to be an eligible medical expense. Eligible w/LMN Penile Implants

A health care professional must provide evidence of medical necessity for the cost of a penile implant to be an eligible medical expense.
